About Pemvidutide
Pemvidutide (ALT-801) is a fatty acid-conjugated, long-acting dual agonist of GLP-1 and glucagon receptors developed by Altimmune, designed for once-weekly subcutaneous administration. Its 48-week Phase 2b MOMENTUM trial in adults with obesity (BMI ≥30) demonstrated approximately 15.6% body weight reduction at the 2.4 mg dose. Notably, the trial produced some of the strongest liver-fat reduction data in the GLP-1 class: 54% reduction in liver fat content as measured by MRI-PDFF, suggesting potent hepatic benefit from its more balanced GLP-1/glucagon ratio. Unlike semaglutide (which has ~300:1 GLP-1:glucagon receptor selectivity), pemvidutide maintains a near 1:1 potency ratio, providing thermogenic and hepatic lipolytic effects that complement central appetite suppression. Phase 3 MASH-specific trials are in planning. Weekly subcutaneous injection. Adverse events are predominantly gastrointestinal (nausea 30–40% in 2.4 mg cohort) and consistent with GLP-1 class. Cardiovascular outcomes data are not yet available. The compound has attracted significant hepatology interest given the MASH epidemic: current estimates suggest 1.5–6.5% of the global population has MASH, with limited FDA-approved treatment options. Pemvidutide's liver-targeting profile may fill a gap that pure GLP-1 agents do not adequately address.
