About Leuprorelin
Leuprorelin (leuprolide acetate; brand names Lupron, Eligard, Prostap) is a synthetic nonapeptide analogue of gonadotropin-releasing hormone (GnRH), FDA-approved since 1985. It is one of the most clinically significant peptides in endocrinology. Paradoxically, while GnRH pulses stimulate LH and FSH release, continuous leuprorelin administration downregulates GnRH receptors on pituitary gonadotrophs — causing a profound and reversible suppression of LH, FSH, and downstream sex hormones (testosterone in males, estrogen in females). In men, castration-level testosterone (<50 ng/dL) is achieved within 2–4 weeks. Clinical indications include prostate cancer (androgen deprivation therapy), endometriosis, uterine fibroids, central precocious puberty, and gender-affirming hormone therapy. In the longevity and performance research space, leuprorelin is studied as a tool for temporary HPG axis suppression followed by controlled restart — relevant to post-cycle protocols and hypothalamic reset in hypogonadism models. A Phase 2 immunological study (TRIIM trial, 2019) used leuprorelin as part of a multi-drug regimen that appeared to reverse epigenetic aging by approximately 2.5 years in a small cohort, generating significant longevity research interest. Available as 1-month, 3-month, 4-month, and 6-month depot formulations, as well as daily SC injection. Side effects of sustained use include hot flashes, bone mineral density loss, and cardiovascular risk consistent with hypogonadal state.
